Introduction: The Lions Cancer Institute Inc. (WA) respects the privacy of its Volunteers and Clients and is committed to protecting personal information provided to it, in application forms, enrolments for training courses, and during skin cancer screening events. This statement explains our policies and practices regarding the use and disclosure of personal information collected by the Institute.
Collection of personal information – Screening Events:
The Lions Cancer Institute collects limited personal information during a skin cancer screening event. Information includes, name, gender, date of birth, current or previous occupation, and contact details.
• (a) All the above information is written by the client on the reverse side of the Release Form, a legal document which must be signed, and witnessed, before a screener will conduct the skin check. The Form is taken by the client into the screening room and results of the skin check (if any) are noted on the Form by the screener. If a Referral is considered necessary a separate Referral Form is completed for the client to take to their GP, to confirm screeners findings, and for treatment if required.
• (b) This data is collected to build up a database of information that may be used by researchers, appointed by the Institute, to analyse whether age, occupation or location contributes to occurrences of any skin cancer found.
• (c) Also, to enable the Institute’s Medical Director to include a request as to the diagnosis by the GP to enable feedback to the screener involved, as a measure of accuracy.
• Requested information on a client’s own, or a blood relative’s, previous skin cancer history, and responses to the seven questions on At Risk Criteria, is information collected to assist the screener when checking the client’s skin.
• Requested skin screening history is collected to enable the Institute to build up a picture of how often people, especially those residing in the regional areas of Western Australia, have their skin checked. This could include remoteness of location, or lack of knowledge of the danger of exposure to the sun by unprotected skin. This information may be used to support any application made by the Institute for Government or private sponsorship funding.
Board Members and Volunteers.
• The Lions Cancer Institute collects from its members and volunteers such information as name, age, gender, residential address, contact details and a photograph (head & shoulders).
General Obligations.
• Disclosure (sharing): Information of a personal nature is not disclosed or made available to any person or organisation outside the Lions Cancer Institute, other than for medical research purposes as defined within the wording of the Release Form.

• Security and Storage of Information: The Release Form is digitally stored in the Lions Cancer Institute’s Web based Management Site, currently Microsoft 365. Data stored on this site is password protected and access permissions are required. Following data entry all forms are professionally shredded. Confidential and personal information such as results of screening events are lodged into a section that has access limited to the Chairman and the Medical Director of the Institute.
• Non- Disclosure Agreements: All Directors, and Volunteers engaged by the Institute, must sign a Non-Disclosure Agreement held by the Institute.
• Access to Information by an Individual: Any person who can prove that the Institute will have a copy of their personal information may request access to that information by contacting either the Chairman or the Medical Director of the Lions Cancer Institute through their contact details provided in the Institute’s website www.lionscancerinstitute.org.au
• Overseas recipients: The Institute will not disclose personal information to overseas recipients.
• How to make a complaint: Any complaints about privacy should be submitted, in writing, or by email, to the Secretary of the Lions Cancer Institute, PO Box 2195, Mandurah DC, WA 6210, or secretary@lionscancerinstitute.org.au
Any complaint received will be dealt with by the Board Executive of the Institute promptly. If a satisfactory outcome is not reached the matter will be referred to the Lions Association of Australia Legal Officer.
