The information we collect is what you voluntarily provide us in the clinic, on the website and by telephone this includes your name, address, telephone numbers and email address. When you attend you will also be required to provide a range of data including your GP details and your medical conditions. If you are under 16years of age we will require your parent or guardians consent to provide us with these details.
This information is used to:
Supply you with treatment in the clinic.
Correspond with your GP or relevant medical professionals with your consent.
Inform you of changes to our services
Send you communications by email, post or SMS that you agree to such as reminders or reviews of appointments with your consent.
We are legally required to store your medical records for 8 years or until 25years old whichever is sooner and they are stored securely. We do not supply your information to any third party.
It is necessary that any information we hold on you is up to date. If you change name, address or telephone numbers please ensure you inform us. You have every right to request a copy of the information we hold about you (this may incur an administration charge)
We take security of your information very seriously by following strict confidentiality policies and procedures this includes protection and securely storing of your treatment records which are kept in locked cabinets and destroyed after eight years(from your last treatment).
