The Purpose of processing Client Data
In order to give professional homeopathy treatments and homeopathy consultations, I will need to gather and retain potentially sensitive information about your health. I will only use this information for informing, homeopathic prescriptions and associated recommendations concerning aspects of health and wellbeing which I will offer to you.
I take basic contact details and information via my website to allow me to contact you and handle bookings.
Lawful Basis for holding and using Client Information
As a full member of the Society of Homeopaths, I abide by their strict Code of Practice and Ethics. The lawful basis under which I hold and use your information is my legitimate interests i.e. my requirement to retain the information in order to provide you with the best possible treatment options and advice
As I hold special category data (i.e. health related information), the Additional Condition under which I hold and use this information is for me to fulfil my role as a health care practitioner bound under the Society of Homeopaths Confidentiality Clause as defined in their Code of Practice and Ethics.
What information I hold and what I do with it
In order to give professional homeopathic consultations, I will need to ask for and keep information about your health. I will only use this for informing homeopathic prescriptions and any advice I give as a result of your treatment. The information to be held is:
• Your contact details
• Medical history and other health-related information (which I will take from you at first consultation)
• Treatment details and related notes (which I will take after each consultation)
I will NOT share your information with anyone else (other than as required for legal process) without explaining why it is necessary, and getting your explicit consent.
How Long I Retain Your Information for
I will keep your information for the following periods:
a. ‘claims occurring’ insurance: (records to be kept for 7 years after last treatment)
b. law regarding children’s records (records to be kept until the child is 25 or if 17 when treated, then 26)
Your data will not be transferred outside the EU without your consent.
Protecting Your Personal Data
I am committed to ensuring that your personal data is secure. In order to prevent unauthorised access or disclosure, I have put in place appropriate technical, physical and managerial procedures to safeguard and secure the information we collect from you.
I will contact you using the contact preferences you give me in relation to:
• Appointment times
• Homeopathy information or information related to your health
• Homeopathic prescriptions
• Special offers and promotions (you may unsubscribe from this at any time)
Your Rights
GDPR gives you the following rights:
• The right to be informed:
To know how your information will be held and used (this notice).
• The right of access:
To see your therapist’s records of your personal information, so you know what is held about you and can verify it.
• The right to rectification:
To tell your therapist to make changes to your personal information if it is incorrect or incomplete.
• The right to erasure (also called “the right to be forgotten”):
For you to request your therapist to erase any information they hold about you
• The right to restrict processing of personal data:.
You have the right to request limits on how your therapist uses your personal information
• The right to data portability: under certain circumstances you can request a copy of personal information held electronically so you can reuse it in other systems.
• The right to object:
To be able to tell your therapist you don’t want them to use certain parts of your information, or only to use it for certain purposes.
• Rights in relation to automated decision-making and profiling.
• The right to lodge a complaint with the Information Commissioner’s Office:
To be able to complain to the ICO if you feel your details are not correct, if they are not being used in a way that you have given permission for, or if they are being stored when they don’t have to be.

THERAPIST’S RIGHTS
Please note:
• if you don’t agree to your therapist keeping records of information about you and your treatments, or if you don’t allow them to use the information in the way they need to for treatments, the therapist may not be able to treat you
• Your therapist has to keep your records of treatment for a certain period as described above, which may mean that even if you ask them to erase any details about you, they might have to keep these details until after that period has passed
• Your therapist can move their records between their computers and IT systems, as long as your details are protected from being seen by others without your permission.

Terms For a Consultation
By booking a consultation with Ann Lovatt you agree to the terms listed below.
Please check that you are happy with all of the details before you proceed with a consultation.
Homeopathy alongside mainstream healthcare:
Homeopathy works well alongside mainstream healthcare programs and medications. You will not be asked to come off medications during your treatment. You may be asked to go for additional health checks to see if your medications are still required.
Homeopathy is not instead of Accident and Emergency Care:
If you need emergency health care please contact your doctor or emergency Services.
Emergency Service:
Ann Lovatt cannot provide an emergency service. If you need emergency care please contact your nearest emergency services.
Booking:
Your agreement will be directly with Ann Lovatt.
This consent form will cover your entire course of treatment.
You confirm that you have made this appointment in good faith and that you have the ability to accept or reject this care of your own free will and choice and are mentally competent to enter into this agreement.
You acknowledge that you are not representing an agency (private, governmental or otherwise) attempting to gather information without so stating (i.e. you are not a reporter posing as a patient).
You understand the charges are to be paid on booking the consultation unless specific prior arrangements have been made or there is no charge.
You will ensure that any photos or videos you send will not show nudity of private parts of the body of adults or children.
You consent to Ann Lovatt holding records and personal information/disclosures that include any information you send & disclosures you make during appointments.
Payment:
When the details are agreed on by both parties, Ann Lovatt will send you, the client an invoice for payment before the consultation begins or immediately after it.
The Consultation:
The Consultation will take place at the time indicated if not immediately.
You will choose between:
A video consult
An internet audio consult, or if travel permits, a consultation in person.
You may be asked for photographs to see skin complaints.
Everything discussed at each consultation is confidential and will not be discussed with anyone unless we have your specific instruction to discuss your information with other health professionals.
Pharmacy:
You will be sent a prescription for your remedies after our consultation. You will be sent details of where to purchase them if you haven’t got the remedies yourself.
Confidentiality :
Ann Lovatt will not share your details. As your homeopath she will keep your records in accordance with UK licencing laws.
Any consultation recording will be held in a secure unit according to the data protection act.
You can ask for a copy of any chat conversation, but please be aware that Ann Lovatt will not be responsible for the security of the email service.
Health Notes:
Ann Lovatt is required by law to keep your consultation notes for 7 years. These will not be shared with anyone unless the law requires her to submit information for the protection of life.
Emails:
When you book a consultation using this service, your name and details will be added to Ann Lovatt's database so that she can send you the prescription.
You can opt out of receiving details about her services, news and courses and about learning more about homeopathy. You can choose to unsubscribe at any time.
Code of Conduct:
Ann Lovatt abides by the Code of Conduct set out by the Society of Homeopaths in the UK.
Technology:
With the use of internet, video calls, messenger, emails or telephone calls, every reasonable precaution possible will be taken to ensure the privacy of your information, but it cannot be guaranteed across these technologies.
Follow Up Consultations:
This will be discussed with Ann Lovatt to ensure the best way forward.
