Our Privacy Policy attests to our commitment to privacy and demonstrates the ways we ensure
that patient privacy is protected. Our Privacy Policy applies to the personal health information of
all our patients that is in our possession and control.
All information disclosed through intake forms, during or after a consultation is confidential, and may not be revealed to anyone without my patient's written permission, except where disclosure is required by law. More information on this will be included in the Patient Intake form and Confidentiality Clause that patients are required to sign before a consultation.
I have my patient’s best interest in mind. I will do whatever I can in my capacity to assist
and protect you. If a patient's spouse or family member contacts me about my patient, I will not even
acknowledge that I know such a person. I will contact my patient and inform that this
person contacted me regarding the client and the patient can then decide what to do with
the information. If the patient wishes, a release of information to enable me to discuss
selective information can be signed. However, it is not required and if signed can be
revoked at any time.
